Student Registration Information

Registration Requirements

If accepted, you will receive an Acceptance Notice.

Students seeking admission for Kindergarten must:

  • Be 5 years old on or before December 31st.
  • Be administered a Kindergarten readiness test.
  • Have a report/evaluation from the pre-school teacher (if applicable).
  • Provide current immunization records.

For all students, the Registration Process includes the completion of the following items: (If these items are not received within two weeks of the enrollment offer, your child may be removed from the school’s accepted list)

  • Registration Form
  • Copy of Birth Certificate
  • Residency Form with two proofs of residency
  • Home Language Questionnaire
  • Provide a copy of child's Physical Exam that has been performed within the last 12 months.
  • Provide a copy of child's Immunization Record.
  • Provide a copy of child's most recent Dental Exam.
  • Application for Free and Reduced Lunch/Breakfast (released August 2017, due to the school by first day of instruction)
  • Copy of current report card
  • Buffalo Public Schools Transportation Request Form
  • Textbook Loan Form

*Other items may be required by administration and central office personnel.

Proof of Residency

Acceptable proof of residency documents must be dated and include the parent/guardian’s name and address. 

The following documents may be accepted:

  • Official driver’s license
  • State- or other government-issued ID
  • School photo ID with date of birth
  • Consulate identification card
  • Hospital or health records (in New York City, Hospital Birthing Records)
  • Military dependent ID card
  • Native American tribal document
  • Record(s) from non-profit international aid agencies and voluntary agencies (VOLAGs) 

Non-Discrimination Statement:  A charter school shall not discriminate against or limit the admission of any student on any unlawful basis, including on the basis of ethnicity, national origin, gender, disability, intellectual ability, measures of achievement or aptitude, athletic ability, race, creed, national origin, religion or ancestry. A school may not require any action by a student or family (such as an admissions test, interview, essay, attendance at an information session, etc.) in order for an applicant to either receive or submit an application for admission to that school.
